Value based proposition
To drive improvement around the value-proposition and performance of the care we provide in our communities, we have developed market specific profiles and scorecards are used to inform and track our ongoing improvement efforts. Some of the key metrics include: quality and patient experience ratings, market share, payer mix, health system utilization of home-based services, capture rates (system integrity), timely initiation of care, readmissions and our level of connectivity with our health system partners.
Adding value to our relationships with ACOs and payers, is one of our approaches to accelerating the growth and innovation of non-acute services provided in the home. This will enhance our ability to deliver high quality, comprehensive care over a broader continuum, and serves as the backbone for patient focused, cost-effective care for patients with chronic and complex conditions.
